A new Tennessee state law requires that we have written or verbal consent from a parent or legal guardian to treat any unaccompanied minor or any minor that is brought in by someone other than the legal guardian.
This includes sick appointments, lab work, and other treatments.
To ensure your child receives the care they need, please complete a consent form that authorizes us to treat your minor child. Once the form is completed, please email it to us at customerservice@
